Competition Application Forms
These application forms are intended for competition teams taking part in the 12th Czech Republic National Extrication Challenge in Chomutov .
A competition team consists of four members – the team captain, a firefighter-paramedic and two firefighters.
By registering the team, the entrant also confirms that the named competitors meet the healthcare eligibility requirements and other conditions set out in the Rules for Rescue Operations.
